Air-Play Release Form

I ( names printed and signed below ) further known as ( ARTIST ) give Adventure Crazy Radio and CISO further known as ( STATION ) permission to air publicly any and all audio sent and/or downloaded to STATION.

The ARTIST understands that any recording sent does not give the STATION the ownership of the song, but only the recording for play on any of the STATIONS streams. This does not give the STATION any rights above any license that is managed by BMI, ASCAP, SESAC, Sound Exchange and/or any other license authority.

The ARTIST understands that any recording sent to the STATION, the STATION has the right to edit, modify, expand, reduce and/or not play an audio.

The ARTIST understands that the audio should be Pop/Classic Rock, but the STATION may consider other audio.

The ARTIST understands that the audio must be a mp3 and be at least 1 hour in length and no longer that 2 hours.

The ARTIST understands that the STATION has the right to remove any foul language or improper discussion from the audio and/or not air the audio.

The ARTIST understands that Adventure Crazy Radio and CISO do not guarantee the airing of any audio sent and retains the right to edit any audio.

The STATION is looking for good band sounds, and interesting open / jam sessions. Audio does not have to be only music, but can be any voice/taking/discussion that my take place in-between songs.

The ARTIST understand that all singers, musicians and/or any one with any dialog must sign this air-play release form and mail or fax it to 209-825-8585. If you have more that 10 people, please use an additional form
